Intake Interview, Case Management and Coordination
Whatever the problem or need that you have as a senior or a caregiver, our Program Coordinator will meet with you at your home and conduct an intake interview to help you to determine what the Block Nurse Program can do to assist you and what resources are available. There is no charge for this interview. The services are tailored to each client; there is no “one size fits all” approach. Each client has a plan best suited to his or her needs. Here are some examples.
In-Home Nursing Services:
Home health care after a hospitalization

Health history and assessment
Vision, hearing and depression screening
Blood pressure and blood glucose monitoring

Foot and nail care

Home Health Aid and Homemaker Services:

The St. Anthony Park Block Nurse Program contracts with the Minnesota Visiting Nurse Program when Nurses, Home Health Aides and Homemakers are needed. Health care visits are billed to Medicare, Medicaid and private insurance as appropriate. For services not covered by insurance, we have a sliding fee scale.
